What Are Ovaries & How Do They Function?

The ovaries are a pair of almond-shaped organs which are located in lower abdomen in the pelvis. There is one ovary on each side of uterus. Further each ovary contains thousands of eggs. Till menopause every month one or more eggs starts to grow in a small cyst like structure known as follicle. When the egg is mature, the follicle ruptures and releases the egg and this process is known as
ovulation. This egg is then carried to the uterus with the help of tube like structures called fallopian tubes.Also ovaries produce two hormones estrogen and progesterone.uterus in case the egg is fertilized and grows into an embryo.

After the rupture of the follicle the ovaries produce progesterone to help the uterus prepare for a fertilized egg. If no pregnancy occurs, the level of progesterone decreases and menstruation occurs.

What is Polycystic Ovarian Syndrome or Disease?

Polycystic ovarian syndrome or disease develops when egg follicles get trapped just under the surface of the ovary and thereby are unable to release their eggs, thus forming into multiple small cysts.

It was originally described by Stein and Leventhal in the year 1935. They described it as a syndrome which has the following features:

-Absence of menstruation (Amenorrhea)
-Increase in facial hair growth (Hirsutism)
-Obesity
-Enlarged ovaries having multiple cysts (Polycystic)

This syndrome is usually more prevalent in the young women in the reproductive period. It is a complex disorder characterized by excessive production of hormones known as androgens by the ovaries and the adrenal glands. Due to this the ovarian follicles fail to mature.
Signs and Symptoms of Polycystic Ovarian Syndrome or Disease
-Increase in the body weight (Obesity)
-Abnormalities of menstruation : The patient can suffer from scanty menstruation (Oligomenorrhea), absence of menstruation (Amenorrhea) and infertility. The patient usually comes with the complaints of irregular menses and says she is not getting her periods for 3-4 months.
-Feeling of pressure or fullness on one side of the lower abdomen.
-Sharp abdominal pain.
-Pain in the abdomen during intercourse.
-Increase in body or facial hair.
-Irregularities in urination (frequent urination) or bowel movements.

On internal examination, the gynecologist usually discovers bilaterally enlarged cystic ovaries. Sometimes due to the excess weight the ovaries may not be revealed.
Diagnostic Procedures in PCOS or PCOD
a) Sonography : Transvaginal sonography is especially useful in obese patients
b) Serum values : Luteinising Hormone (LH) levels or the ratio between LH : FSH (Follicle Stimulating Hormone is > 3 : 1. There is usually reversal of Oestradiol : Oestrone ratio. The Oestrone level is markedly low. Androstenedione is elevated. Serum testosterone and DHEA-S may be marginally elevated.
c) Laparoscopy : Bilaterally polycystic ovaries are detected which is the characteristic feature of PCOS or PCOD.
Diet & Nutrition in PCOS or PCOD
A vegetarian diet is recommended especially a diet rich in foods that naurish the liver.
