After the rupture of the follicle the ovaries produce progesterone to help the uterus prepare for a fertilized egg. If no pregnancy occurs, the level of progesterone decreases and menstruation occurs.
What is Polycystic Ovarian Syndrome or Disease?
Polycystic ovarian syndrome or disease develops when egg follicles get trapped just under the surface of the ovary and thereby are unable to release their eggs, thus forming into multiple small cysts.
It was originally described by Stein and Leventhal in the year 1935. They described it as a syndrome which has the following features:
-Absence of menstruation (Amenorrhea)
-Increase in facial hair growth (Hirsutism)
-Obesity
-Enlarged ovaries having multiple cysts (Polycystic)

a) Sonography : Transvaginal sonography is especially useful in obese patients
b) Serum values : Luteinising Hormone (LH) levels or the ratio between LH : FSH (Follicle Stimulating Hormone is > 3 : 1. There is usually reversal of Oestradiol : Oestrone ratio. The Oestrone level is markedly low. Androstenedione is elevated. Serum testosterone and DHEA-S may be marginally elevated.
c) Laparoscopy : Bilaterally polycystic ovaries are detected which is the characteristic feature of PCOS or PCOD. |
